Today we submitted an application to be a third party contributor to Google Health. We have high hopes of being successfull in our pursuit.
How it would work:
1) Gather all of your medical records from your physicians.
2) Fill out and send this consent form along with your medical records to:
P2E, 216 River Street, Haverhill, MA 01832
3) We will contact you as soon as the records are recieved and provide you with a password for our encrypted web portal where you will be able to view and download your scanned files.
4) All files will be scanned at 300 DPI Color and run through our OCR software that will allow you to copy and past relevant information.
5) You will be billed by electronic invoice your total cost before you will be able to download your files.
Total Cost = $15 setup fee + 0.15 cents per page.
6) We ship you your medical records along with a back up DVD free of charge.
